Introduction

To qualify for the RM15,000 annual FTES living allowance, a student’s gross household per capita income must not exceed RM1,500 per month. While parents with salaried employment can easily submit formal monthly payslips or EA tax forms, families in the informal or agricultural sectors often do not have fixed payroll records.

If your parents are self-employed, smallholders, gig workers, fishermen, or housewives, you must submit an official Borang Pengesahan Pendapatan (Income Verification Form) endorsed by a recognized local authority.

Who Needs to Submit an Income Declaration Form?

Submit this official form if either parent or guardian falls under the following categories:

  • Farmers, oil palm smallholders, fishermen, or agricultural workers.
  • Hawkers, petty traders, or home-based business owners.
  • Grab/e-hailing drivers, delivery riders, or freelance contract workers.
  • Unemployed, retired without pension, or full-time homemakers.

Who Is Authorized to Endorse the Form?

🏛️ Important: Income declaration forms cannot be self-certified. To be legally accepted on the my-Yayasan portal, the form must carry an official signature and rubber stamp from an authorized authority:

  1. District Officer (Pegawai Daerah) or Assistant District Officer.
  2. Community Leaders (Ketua Masyarakat / Keturunan): Pemancha, Penghulu, Kapitan, or Tuai Rumah.
  3. Village Development & Security Committee (JKKK) Chairman.
  4. School Headmaster / Principal (where the student or younger sibling currently studies).
